This project expects to define and apply the Person-Centred Comprehensive Attention model (ACP) in home care services, promoting the stay of people in their homes. The stages of the project are the following:

• Demarcation and diagnosis of the situation in the territories where the model will be applied: socio-demographic map, inventory of socio-sanitary resources, etc.

• Planning of the actions to do in order to apply the ACP model in home care services: coordinated socio-sanitary care, training, service supply in homes, advanced remote care, adaptation of the housing, respiratory care, etc.

• Implementation of the new model to the key figure of the case agency.

• Evaluation of the implementation of the ACP model in home care services, identification of the Good Practices and training of professionals.
